To ensure your visit is efficient and personalized, we require two items before scheduling a consultation or surgical appointment:

1. Referral from Your Dentist
Most appointments require a referral from your general dentist or specialist. Your dental office may send this directly to us, or you may upload it as part of the registration process.

2. Completed New Patient Intake Form
Please complete our secure online new patient intake form. Once submitted, your information is automatically added to your chart, allowing our team to review everything and contact you promptly to schedule your appointment.

After You Submit

Once we receive your referral and completed intake form:

  • Our clinical team will review your information

  • A patient care coordinator will contact you to schedule your appointment

  • You’ll receive guidance on next steps, including any imaging or records needed
